Visitation Update: May 28th, 2021

Dear Patients and Families:

I am writing to share an update regarding visitation at Chase City Health & Rehab Center. We understand this has been a challenging year for both our patients and their loved ones. We are excited to expand our visitation options while maintaining the safety of our patients and staff in accordance with the most current visitation guidance for nursing homes from the Centers for Medicare and Medicaid Services.

Effective June 1, 2021, we will begin offering in-room visits for fully vaccinated patients and visitors, indoor visitation in designated areas, and continuing our outdoor visitation schedule.  Virtual and compassionate care visits will also continue to be available.

Important information:

  • All visitation must be scheduled at least 24 hours in advance. Scheduling is on a first come, first served basis by utilizing our online scheduling portal or by calling the Social Worker.
  • Initially, we ask that you space out your indoor visits to allow the opportunity for all patients to have a scheduled visit.
  • Visits will be scheduled for 30 minutes in duration.
  • Indoor visits will be available 7 days a week between the hours of 1:00 pm and 6:00 pm. Outdoor visits will be available every Thursday through Saturday between the hours of 1:00 pm and 5:00 pm. 

Note: We are offering indoor visitation only on Sundays.

  • The number of visitors per patient at one time and the total number of visitors in the Center at one time must be limited. Each patient is allowed two visitors per visit. Visitors must be age 10 or older.
  • We ask that visitors who have received the COVID-19 vaccine bring proof of vaccination to their initial visit.
  • We ask that you refrain from eating/drinking during visits in any location of the center. 
  • We will continue to screen all visitors for signs and symptoms of COVID-19 including a temperature check and screening questions. Any visitor who is experiencing signs or symptoms will not be allowed to visit.
  • For vaccinated visitors and residents, face masks may be removed during the visit; however, you must wear a face mask while in common areas of the center. Unvaccinated visitors must wear a face mask appropriately (covering nose & mouth) for the duration of the visit.
  • Social distancing of 6 feet should be maintained from staff, other patients and visitors.
  • Hand hygiene must also be utilized before and after each visit.

Visitors who fail to adhere to these core principles will not be permitted to visit and will be asked to leave.

The safety of our patients is the responsibility of the Center. Staff members are required to monitor visitation to ensure the core infection control principles are observed at all times while offering as much privacy to visitors as practicable. By following these precautions, we are confident that we can safely facilitate visitation inside the Center.

Testing

Should the Center be required to initiate outbreak testing, visitation will be temporarily suspended pending the outcome of the testing. If the county positivity rate increases to more than 10%, we will evaluate the current percentage of patients who have been vaccinated and communicate any changes to visitation accordingly.

COVID-19 testing will be available for any visitor participating in indoor visitation. Appropriate consent forms must be completed prior to testing. If you would like to be tested prior to your visit, please make the Social Worker or Receptionist aware and arrive 15 minutes prior to your scheduled visit to allow time for the testing process. Testing will continue to be required for all compassionate visits regardless of vaccination status.
