Dear Patients and Families:

I am writing to share news that a staff member has tested positive for COVID-19. This individual will remain off from work until the CDC’s return to work criteria have been met. We have initiated outbreak testing, which will occur every 3-7 days until the Center is 14 days without a positive result.

Due to the increasing community positivity rate and the Center’s current outbreak status, the following temporary changes to visitation are effective immediately:

• Visits must be scheduled in advance using our online scheduler: https://chasecity-rehab.com/patient-visitation/
• Visits will occur in the lobby or outdoors.
• While compassionate care visits may occur at any time, we request that you provide as much advance notice of your visit as possible.
• As a reminder, any patient who is placed on transmission-based precautions will be limited to virtual or window visitation until precautions may be safely lifted.
